  • LiveScans are by appointment only. No walk-ins.
  • Address: 1155 Market Street, 4th Floor, San Francisco, CA 94103
  • You must complete orientation and have a 9-digit IHSS ID number assigned from IPAC.
  • Must work in San Francisco County.
  • During the Live Scan appointment you must bring:
    • Request for Live Scan Service Form (BCIA 8016 Form)
    • A valid government picture ID cards.
